Marlboro County General Hospital Loan Repayment Program

Please complete the application in order to be considered for the Marlboro County General Hospital Foundation Loan Repayment Program. You will be asked to upload your curriculum vitae, a community letter of support from a lay member of the community, copy of the SLED State Criminal Records Check (there is a minimal charge for this check.), completed National Practitioner Data Bank Self Query (there is a minimal charge for this check), and copies of all educational loan documents in order to submit your application. You will also be asked to provide an email address for a site administrator who will be sent an employment verification form to complete. 

We will not accept any applications or documents via email, mail or fax. In order for your application to be considered complete, you must upload all required materials to your application in Submittable.   You will have the opportunity to revisit your application and continue to update it until submission. Applicants will receive confirmation after submitting their application and can view application status at any time in their Submittable account.
