Please complete the application in order to be considered for the South Carolina Rural Incentive Grant Program. All application materials must be submitted through the online portal by March 15, 2024 at 4:00 pm EDT to be considered. You will be asked to upload your curriculum vitae and a completed Practice Site Profile Form in order to submit your application. We will not accept any applications or documents via email, mail or fax. In order for your application to be considered complete, you must upload all required materials to your application in Submittable.

You will have the opportunity to revisit your application and continue to update it through March 15, 2024 at 4:00 pm EDT. Applicants will receive confirmation after submitting their application and can view application status at any time in their Submittable account.
