Please complete the application in order to be considered for the South Carolina Rural Behavioral Health Professional Incentive Program. All application materials must be submitted through the online portal by April 11, 2025 at 4:00 pm EDT to be considered. We will not accept any applications or documents via email, mail or fax. In order for your application to be considered complete, you must upload all required materials to your application in Submittable.

Proof of U.S. citizenship or U.S. national status:
- U.S. birth certificate issued by a city, county or state agency of the U.S., identification page of unexpired U.S. passport, Certificate of Citizenship, or Naturalization Certificate (Note: state driver’s license, state identification cards, residence cards and Social Security cards are not acceptable proof.)
- Site and Employment Verification Form
- Participant CV
You will have the opportunity to revisit your application and continue to update it through April 11, 2025 at 4:00 pm EDT. Applicants will receive confirmation after submitting their application and can view application status at any time in their Submittable account.
